Integrated health systems are considered part of the solution to the challenge of providing care with quality and efficiency. Healthcare integration allow transfer of relevant information from one caregiver to another, reducing the conflict of clinical protocols, as well as increasing learning capacity over patients. Simultaneously, integration at different levels of healthcare, promotes access and reduce costs. NOVAhealth Integrated Care group is committed to contribute with research activities on the field, to implement innovative care integration with gains for the health of population. For that purpose, we plan to use organizational, digital and health literacy instruments, to access use, costs and outcomes and, currently, trying to respond to the following research question: How can multiple responses be integrated in a given diabetic population?
